Summary/Objective: To advance the independence, productivity, and full citizenships of persons with disabilities.

Essential Functions: Provide physical and technical assistance and instruction to adults with disabilities.

Qualifications: High school diploma. Background in visual arts, performing arts and/or technology and a willingness to learn. A valid California driver’s license and proof of insurance, or means of personal transportation.

Job Duties and Responsibilities:

  • Protect client rights, health, safety and well being
  • Advocate for the rights of the student
  • Assist students with self-advocacy skills
  • Develop lesson plans and schedules
  • Assist with basic care needs
  • Provide input in the development of individual service plans
  • Maintain open, professional communication with supervisor and UCPCC
  • Attend workshops, conferences, and in-services for the purpose of professional growth
  • Attend staff meetings
  • Other duties as requested

Physical Requirements:

The following is a description of the physical demands of the job:

  • Must be able to lift 50 lbs. and carry loads of 50 lbs. for short distances.
  • Must be able to bend, sit and stand for long periods of time.
  • Must be able to view a computer screen.
  • Must be able to endure extended work periods in environments that include varying climates both cold and hot.
  • Fine and gross motor skills for equipment/technology use and maintenance.
  • Must be able to physically drive a vehicle per the standards of the CA OMV.
  • Must be able to physically push and move persons in wheelchairs.

Supervision Received: Reports directly to the Director of Adult Program Services

Classification: Non-exempt

Salary Range: Negotiable

Schedule: Monday-Saturday, varied hours per week. Exact schedule to be determined by Home Adaptive Supervisor and Program Manager.

Special Working Condition: Programs are usually implemented in the client’s home adaptive area in the UCP affiliate’s region (Fresno, Kings, Tulare, Merced, Madera, Mariposa). The job requires a considerable amount of driving time (to and from the client’s residence) with mileage reimbursement. Employees may not discuss confidential client information outside approved professional meetings and conferences.

Education and Experience: High School Diploma. Experience working with developmental clients is preferred.

Personal Qualifications: Must be a self-motivated, conscientious individual. The job requires a considerable amount of self-supervision. Patience and flexibility are important. The ability to communicate objectively with clients, parents, care providers, and other agencies. Ability to give and receive professional, constructive criticism. Good oral and written communication skills. Professionally assertive, tactful, responsible, flexible, with a sense of humor. Good physical health and strength. Good grooming and neat appearance (Personal Appearance and Conduct section of the employee handbook).

Job Objectives: To provide appropriate program continuity at the home adaptive, workplace, or school of clients who are unable to attend a program at the UCPCC center or another community-based program.

Duties/Responsibilities:

  • Implement programs as written by the supervisor.
  • Programs may include: gross motor exercise, basic education, adult development (art and crafts, self-help skills, mobility training, activities of daily living), Independent living skills, communication skills, recreation, adapted physical education, personal rights, advocacy, instruction to parents or care providers, assistance in procurement of equipment, and instruction of use of the same.
  • Write quarterly evaluations and submit them to the Home Adaptive Supervisor.
  • Write progress notes after each individual Home Adaptive direct service.
  • Record mileage and hours after each individual home adaptive visit.
  • Communicate with CVRC, UCP regarding client needs as necessary.
  • Consult with Home Adaptive Supervisor, Program Director, therapists, Adaptive P.E. Specialist regarding client progress.
  • Contact/confer with parents or care providers when necessary, regarding client information and program.
  • Log phone calls, client-related expenses, such as approved program supplies, which may be necessary for program implementation.
  • Attend in-services as required.
  • Provide any other documentation as requested by Home Adaptive Supervisor, therapists, or Program Director.
  • Perform any other job-related tasks as requested by Supervisor and Program Director.

Objective:   To provide parent-child developmental services in a home environment or other natural environment setting for children and their families who qualify for Early Start services.

Qualifications:

  1. ECE required course work or the ability to do so in a reasonable time frame.
  2. Familiarity with wide range of developmental disabilities and services for children with disabilities.
  3. Ability to maintain effectively, respectful, culturally sensitive interpersonal relationships with staff, families, and volunteers.
  4. Ability to work with parents of all ages.
  5. Valid California Driver’s License or reliable transportation AND proof of insurance.
  6. Ability to work as a team.
  7. Ability to maintain good relationships with community partners.

Desirable:

  1. Experience with young children and families.
  2. Experience with children with special needs.
  3. Bilingual.

Job Duties and Responsibilities:

  • Model appropriate child development practice while providing a minimum of weekly services in a home or other approved natural environments.
  • Participate in required early intervention service provider trainings.
  • Work respectfully and collaboratively with all disciplines of UCP staff,
  • families, partner agencies and the public. Be a team player.
  • Learn and implement culturally sensitive and developmentally appropriate curriculum to meet the individual developmental needs of infants and toddlers.
  • Model appropriate parenting skills and coach parents in their child’s early intervention program in the home and/or classroom environment.
  • Provide resource and referral for parents and children as appropriate.
  • Screen infants and toddlers for disabilities or delays.
  • Prepare present levels, outcomes, and other needed documentation for ISFP meetings and attend those meetings as a representative of UCP.
  • Participate in team meetings and trainings.
  • Maintain good interagency relationships.
  • Complete daily individual chartings, developmental charts, referrals and paperwork with HELP charts, and other paperwork as assigned.
  • Maintain service provision environment in good operating order including set-up, cleaning, and toy sanitation.
  • Prepare activities that reflect the needs of children.
  • Provide respectful, appropriate parent support when needed.
  • Participate in reflective supervision with other staff and program management.
  • Other duties as required.

Work Hours: Hours as needed for the program.  travel will be required.

Salary /Benefits: This is an hourly position. Health/Life Insurance after the first 90 days of employment.  Dental/Vision plan is available at employee’s expense.  Dependents may be covered on health/dental/vision plan at the employee’s expense.  Ten (10) paid holidays plus two (2) personal days (coded as vacation) per year are provided.  Vacation and sick accrual start after the first 90 days of employment.  Section 125 Plan available.

$18-$21 per hour, hours per week to be determined.
